A Personal Reflection Tool for Pastors and Ministry Leaders

Complete this assessment prayerfully and honestly. Choose the option that best represents your current health in each area.


This tool is designed to help you slow down, reflect, and identify areas where you may need renewed attention, healthier rhythms, or trusted support. It is not meant to create shame, measure your worth, or evaluate your calling.

How to Score Each Area


For each category, choose the number that best describes your current health.

1 — Needs Attention


This area is strained, neglected, unhealthy, or creating concern.

2 — Growing


This area has some healthy patterns, but still needs intentional attention.

3 — Thriving


This area is generally healthy, life-giving, and sustainable.

1. Spiritual Vitality

Reflects a deepening relationship with God through prayer, Scripture, obedience, worship, and dependence on the Holy Spirit. Prioritizes being with Jesus over ministry output.

2. Emotional & Relational Health

Practices self-awareness, emotional honesty, humility, and healthy relationships. Manages conflict with grace and does not ignore emotional strain, resentment, loneliness, anger, or discouragement.

3. Marriage & Family Health

Gives proper attention to spouse, children, family relationships, and life at home. Does not consistently sacrifice family health on the altar of ministry demands.

Demonstrates a commitment to ongoing growth, wisdom, maturity, and self-leadership. Invests in learning, mentoring, coaching, reading, reflection, and personal development.

Practices sustainable rhythms of ministry, rest, Sabbath, renewal, and life outside of church responsibilities. Makes space for friendship, hobbies, recovery, and meaningful life beyond ministry work.

Leads others toward spiritual maturity through teaching, mentoring, example, and intentional discipleship. Encourages people to grow in faith, obedience, service, and transformation.

Score Interpretation

29–33 — Thriving


You appear to have strong health and sustainable rhythms in many areas. Continue reinforcing what is working, stay attentive to early warning signs, and consider how you can encourage other pastors.

23–28 — Healthy and Growing


You have several healthy patterns in place, but one or two areas may need intentional attention. Choose a specific area to strengthen over the next 30–90 days.

16–22 — Needs Attention


Some areas of strain may be affecting your overall health, leadership, relationships, or ministry sustainability. Consider making adjustments, talking with a trusted person, and seeking support.

Below 16 — At Risk


Your ministry load may be outweighing your current health rhythms. This is not a statement about your calling or worth. It is an invitation to seek support, slow down, and take meaningful steps toward restoration and health.
